Personalised cancer vaccines reach a pivotal milestone. Manufacturing is the next challenge.

INTerpath-001 validates the clinical potential of an individualised neoantigen mRNA therapy in resected high-risk melanoma. Scalable personalised manufacturing remains a central opportunity.[1]

The clinical results

The first positive phase III readout for an individualised neoantigen mRNA cancer therapy in this setting is a major milestone: a clinical principle is gaining validation, while a new industrial chapter opens.[1]

Sensitivity analyses: a different lens, not a new primary result

In the prespecified piecewise analysis after six months, RFS HR was 0.42 (95% CI 0.30–0.58) and DMFS HR 0.50 (95% CI 0.34–0.73). During the first six months, RFS HR was 0.99 and DMFS HR 1.01. These analyses do not replace the primary intention-to-treat results.[1]

Secondary reading: modified intention-to-treat (mITT)

Among participants receiving at least one vaccine or placebo dose, the prespecified mITT sensitivity analysis reported RFS HR 0.51 and DMFS HR 0.55. This selected population is distinct from the primary ITT population.[1]

Overall survival (OS) remains immature; a statistically established OS benefit has not been demonstrated. The ESMO 2026 presentation reports HR 0.77 (95% CI 0.45–1.31), p=0.33. Follow-up continues.[3]

Why neoantigens?

Each tumour has a distinctive mutation profile. Tumour and normal sequencing, HLA typing and computational selection identify candidate neoantigens. In this product, mRNA encodes up to 34 neoantigens; lipid nanoparticle (LNP) delivery supports T-cell priming. Anti-PD-1 therapy complements this strategy by reducing checkpoint-mediated inhibition.[1]

Manufacturing & access

01

A multi-step personalised process

Sampling, sequencing, selection, production, quality control and logistics all contribute to turnaround. The ESMO 2026 presentation reported a median 64 days (about nine weeks) from the start of pembrolizumab to the first vaccine dose—not manufacturing time alone. Designing for shorter turnaround is an opportunity, not a proven clinical benefit.[3]

02

Tissue quality starts upstream

The patient flow presented at ESMO 2026 reports 302 of 1,749 screened patients (17.3%) with an inadequate tumour sample. This is not a 17.3% manufacturing failure rate among the 1,137 randomised patients. The ESMO 2026 presentation reports only one patient whose vaccine could not be manufactured, who received placebo to maintain blinding.[3]

03

Complexity, cost and access

A unique medicine for each patient creates production and access challenges. No verified commercial price is available here. Health-economic assessment and real-world feasibility remain open questions; established unaffordability cannot be inferred.[1]

04

Clinical questions remain

Overall survival, predictive biomarkers, mechanism of action, long-term quality of life and positioning versus neoadjuvant ipilimumab/nivolumab need further study. There is no head-to-head comparison and no superiority conclusion against that strategy.[3]

Data presented at ESMO 2026 · combination / control[3]

Data presented at ESMO 2026 report treatment-related grade 3–5 events in 14.2% versus 13.4%, and serious treatment-related events in 9.8% versus 11.1%. Fever, injection-site pain and fatigue were common. Similar severe-event rates do not mean risk-free treatment.[3]

The ESMO 2026 presentation reports two treatment-related deaths in the combination arm versus none in control. Both were reported during the presentation to have occurred before the first vaccine dose and to have been attributed to pembrolizumab.[3]

Overall-survival and long-term quality-of-life follow-up are ongoing.[1]

Innovation in the manufacturing process

These results mark a turning point for personalised cancer vaccines. Building on this principle, now clinically validated in melanoma, OncoRNA is developing a process innovation based on synthetic DNA, designed to shorten and simplify personalised RNA production. A potentially transformative approach to making this new generation of treatments more accessible.

The OncoRNA platform is in preclinical development.
